SAIL Fall Prevention Program

The Strategies and Actions for Independent Living (SAIL) fall prevention program is an evidence-based program designed for home health professionals who provide support for clients receiving home support services as well as offering downloadable resources directly for clients.

The SAIL Home Activity Program (HAP) offers both training for home health professionals and resources for clients. The Home Activity Program is made up of seven simple exercises for frail seniors and adults with significant physical disabilities to promote health and well-being and to prevent falls. The three different levels (sitting, standing and moving) offer options to increase physical activity with frail older adults.

In addition to the HAP information, the SAIL fall prevention program includes downloadable resources such as a Fall Risk Screen brochure, a fall prevention multimedia package aimed at engaging both seniors and their doctors and tips for seniors on avoiding falls.
